If you have a child that you are concerned about AND the child is already on an IEP, please fill out the "Teacher Observation Form" below and return to the coop.
If you have a child you are concerned about and they are NOT on an IEP, then you MUST go through your building's SIT team, attempt interventions and the meet with the SIT team again before filling out the form.
